NIKKO TOURAn excursion to NIKKO will take place on Oct. 27th. NIKKO is one of the most famous tourist attractions in Japan with beautiful tinted autumnal leaves. Please see Excursion to NIKKO. NIKKO TOUR is subject to cancellation with less than 30 persons.
APPLICATIONTo apply, please print Registration and Hotel Application Form on your printer. Complete and return it by fax or mail to Nippon Travel Agency (NTA), International Travel Department no later than September 30, 1999. Please use one sheet for each applicant. Please TYPE or use CAPITAL LETTERS. Reservations are confirmed when you pay the total hotel charge. Please note that total room charge will be deducted by NTA prior to check-in date. Please send NTA a copy of your card, front and back. If you wish on site payment for room charge at the hotel, regular room charge may be used.

CONFIRMATION OF REGISTRATIONUpon receipt of the registration from by airmail or fax, a confirmation notice of registration, hotel accommodations and NIKKO TOUR will be sent either airmail or fax. Please bring the confirmation notice with you to the conference.
CANCELLATION AND REFUNDSSubsequent to receiving confirmation of hotel accommodations, cancellations are subject to the following charges, bank charges will be added to said cancellation charge and deducted from the refund.
Until 2 October, 1999 ------------------------------------------------- No charge
From 3 October to 9 October,1999 ---------------------------- 10% of one night room charge
From 10 October to 3 days prior to check in date ----------- 20% of one night room charge
2 days to 1 day prior to check-in date --------------------------- 50% of one night room charge
Reservation day or no show ---------------------------------------- 100% of one night room charge
PASSPORT AND VISAAll foreign visitors to Japan must have a valid passport. It is the responsibility of all participants to obtain all necessary documents, including a visa if necessary, for entry into Japan. Please contact the DD12 Local Organizing Committee dd12@applmath.tg.chiba-u.ac.jp in the case that you should get a visa.
